This QR code is for internal use only. It is available only to patients from hospitals or health centres that are part of the "Virtual Wards" programme. As part of the signing up process, you will be further required to enter a second 4 to 9 digit alphanumeric code known as your hospital Virtual Wards code.

Joining The Programme

Joining the programme is a simple 3-step process:

Scan the download QR code on the left with the camera of your mobile phone.

Download the telemetry app and click on the app to open it. Complete the two forms titled:

- Patient's Registration Form
- Telemedicine Consent Form
You will receive an SMS or email confirming that your profile has been approved by your Clinical Team.
Pair your smart device with your mobile phone to complete on-boarding.
